Alzheimer's

Alzheimer's disease is associated with impaired cerebral glucose metabolism. Ketones may provide an alternative fuel signal, but clinical evidence remains early and heterogeneous.

Scientific rationale

  • Ketone bodies may partially bypass impaired glucose utilization and may also influence mitochondrial function, oxidative stress, inflammation, and synaptic signalling.
  • Interventions studied include ketogenic diets, MCT oil, ketone esters, and other exogenous ketone strategies, each with different adherence and safety profiles.

Research snapshot

  • Small clinical trials and reviews report cognitive or functional signals in some participants, but sample sizes, duration, outcome measures, and genetic factors vary.
  • The field still needs larger trials that distinguish symptomatic support from disease modification.

Clinical frame

  • Older adults with cognitive impairment are vulnerable to weight loss, medication complexity, and nutritional deficiency, so unsupervised restrictive diets can be risky.
  • Ketogenic strategies should not be presented as a proven dementia treatment.

Open questions

  • Which disease stage, APOE genotype, metabolic profile, or cognitive phenotype is most likely to respond?
  • Can benefits be achieved with less restrictive ketone strategies that preserve nutrition and quality of life?
